- Please download Preparticipation Physical Evaluation (PPE) pages 1, 2, 3 and 4.
- Parent and athlete to both complete forms (Page 1, 2), including important medical information pertaining to your student athlete that the school will need to know.
- Bring all four (4) pages to the licensed practioner for physical examination in timely manner prior to sports season.
- Student or parent shall submit ONLY PAGE 4 to the school athletic office as part of the athletic registration process.
